Procedures

Instructions for use of the Enforcement Portal:
  • Enter the information requested in each section of the form as completely and accurately as possible.
  • You will have the option to save the complaint to complete at a later time on each page of the form by clicking on the Cancel/Exit button at the bottom of the page; you may access the form later with the reference number provided.
  • Once the complaint form has been completed and submitted, the form cannot be access and no changes can be made. Please contact the Board office if you have additional information.
  • You will have the option to upload supporting documentation at the end of this wizard.
